Launch of a new version of the application form for dossier upgrading

The Agency for Medicinal Products and Medical devices (HALMED) is informing marketing authorisation holders about the new version of the application form for dossier upgrading, that should be attached to the request for dossier upgrading.
This application form (.doc, .pdf) is accessible in the Croatia in the EU/Dossier upgrading/Instructions for submitting applications for dossier upgrading section of the webpages or here.
